6 Hidden Roles of a Good Physical Therapist

Long ago, before complexities of health care became the templates, movement was the standard medicine. History narrates that early physicians used their hands, heat, and their knowledge of human movement to cure physical dysfunctions. Through the years, the health profession evolved and diversified into highly-specialized components and subcomponents. Nevertheless, principles of these ancient arts of healing had to be passed on for people of today to benefit from. Physical therapists, physiotherapists or PTs are some of the lucky heirs. Arts were thoroughly transformed into science highly influenced by western medicine. Therefore, it is through PTs that people can taste the standard medicine our ancestors used to benefit from, adjusted to suit the modern times.

Every 8th of September, PTs all over the globe celebrate World Physical Therapy Day. I took this time to reflect on the past years and ponder on what I have been doing on a professional level. Years have been great! Nothing can be more rewarding than witnessing a positive change in another person’s life. It is tough to be an agent of change, and to be a good one, I realized that PTs must assume different roles to excel in their field of practice. In my opinion, below are six roles that we unknowingly assume to be good and effective physical therapists.

Counselor

Physical disability results in a great psychological tumult. Movements that used to be reflexive like reaching for a glass of water or standing from a chair become unsurmountable for a person who had a stroke or survived a car accident. Imagine the numbness you feel when your dentist anesthetizes a side of your jaw making you impossible to gargle water without spilling. Magnify that towards half of your whole body – that is a taste of what stroke feels like. PTs must be mentally and emotionally strong to be the patient’s shrink throughout the process. All the anxiety must be transformed to motivation. I remember one time I treated a patient with stroke and taught him how to sit again on his own. He did it well and it was his birthday then. He cried. I held my tears. I had to.

Historian

Aside from being familiar with the ancient origins of modern practice, a PT’s inner history buff must be translated into practice. Knowing a patient’s history of current illness, medical history, family medical history, occupational history, social history and personal history will paint a picture of a whole person in different facets. The uniqueness of this person before an injury would be our benchmark. We will try our best to help our patients assume their best possible self. A good PT will remember every small progress a patient makes. As historians or storytellers fond of beautiful tales, the rehabilitation process should make a pretty good story for the patient to tell when the life hurdle has been overcome.

A PT interacts with a patient for an hour or more, for weeks, months or years for some. Hopeful family members look up to their father or mother or child’s PT to facilitate the healing process smoothly. PTs must also be the strength of the family because a major illness of a family member affects most or everyone. Thus unknowingly, PTs become integrated into the patient’s family at least within the duration of care. This reminds me of a time when I was treating a Korean patient. I had to learn basic phrases in their language as he cannot speak or u
nderstand English. Before we parted ways, when he already started walking, he left me a note. He probably thought I was his grandchild. More than anything, I was amused and flattered at the same time.

Nurse

Health care is a very challenging profession. Like nurses, PTs have to be in constant communication with doctors and other health professionals to deliver holistic care. We have to attend to our patients’ needs in a very compassionate manner. It is a different job altogether as we do not administer medicines, invasive procedures and do graveyard shifts, but we share the nurses’ sincere concern for their patient’s well-being.

Gym Coach

As advocates of movement, exercise is the core of every physical therapy session. As ambassadors of fitness and wellness, a good PT facilitates a patient’s transition to a healthy lifestyle. Exercises specific to body areas warranting movement are carefully administered in well-thought-of dosages. As a patient recovers, PTs help the patient assume a healthier lifestyle to function well and to prevent recurrence of their injury.

Every kid loves the cool teacher – that unique one who can help you learn the most boring class in a fun way. If a teacher can be cool, he or she will forever be a positive memory to a student who acquired the love to learn. PTs are educators too. Extroversion is a helpful trait as this profession requires loads of human interaction. A good PT loves people. He or she channels all negativities into thoughts of hopes and potentials. A good PT ensures the family that whatever it is that they are facing is only temporary. He or she makes the healing process a fun and positive experience, something to look back
to once everything has been overcome.

Physios are agents of change, they say. Indeed having different roles every day, without them knowing, PTs CHANGE lives every single day. To whatever brought me here, thank you. I am proud and will always be. Happy World PT Day!

45 Days of Fresh Greens and Red Rice Diet and its immediate effect on body composition

Perfection.

Without rice, most Filipinos would not consider a meal complete. There are various degrees of rice consumption: from a cup, half a cup, a cup and a half, to the most popular extra rice, and the “barako” (strongman’s) heap. No judgement please, I’ve been in all degrees at different stages in my life. At this point though, I decided to try out an alternative version which started to gain popularity recently with the rise of consciousness towards the pursuit of a healthy lifestyle.

I substituted two of my full meals with red rice meals and occasional pasta. This happened through my friend Cobi Ochoa, whose experience in nutrition and his family’s tradition of preparing gourmet dishes on a regular basis got me in as their first regular client. Mondays thru Fridays, freshly-prepared healthy meals arrive for me right at our workplace, F45 Training BGC Stopover. Hassle-free food service with good quality and definite cost-effectiveness is hard to find. I am lucky!

With goals of mass-gains for ectomorphs (or hard gainers, skinny guys, scarecrows) like me, protein is a staple. Too much protein however could result to constipation, which then warrants a good source of fiber – that is red rice. Perfect combo indeed: Cobi’s high-protein dishes with fibrous red rice. Bullet-proof, nothing to lose. Fire away, fire away. Okay I’ll stop there.

Here are my body composition measurements in a span of 45 days:

With my weight-gain goals, this diet served its purpose. Cobi and I were clear on this goal so he personalized my food proportions. Increase in fat percentage is understandable at this stage since I am building mass. Alongside change in diet, daily exercise for 40 – 60 minutes has become part of my lifestyle especially since I became part of F45. Indeed, diet and exercise must go along. For me, it went well. I shall do this more then!

Neuroanatomy was my favorite subject back in college. Not that I got good grades, I simply enjoyed it. This became my inspiration for drawing “Afferent, Efferent”, an artwork that was recognized by the World Confederation for Physical Therapy (WCPT) as a runner-up in the recent Art and Health Competition held in Singapore last May 2015.

Having this recognition further affirmed my belief that I can be both a left brainer (as a physical therapist) and a right brainer (as an artist of all sorts) and still be human. Even better, my swollen ego can now have a solid foundation. I might be some great person who just become even greater. *shrugs shoulders and smirks* Haha. Sorry guys. I kid. 50% kid. :p

If you think things do not make sense, take the chance to make sense out of them. Take every chances to defy the status quo and combat mediocrity. That’s what I try to do. Stand out. Think. Exist.

Ambitious, competitive, outspoken. These words describe me. It is tough to have that “I am great” mindset and grow up (sometimes way too up. lol) in an environment that proves you otherwise. Apparently, the world is bigger than you and you need to be reminded of that. In every phase of my life, something or someone is put in front of me to remind me that I am not perfect. Megalomaniacs, people who think too big of themselves, i.e. people like me, find it challenging to take in situations where in we have to keep our feet on the ground and accept reality.

Late last year, I  decided to try my luck in the field of ramp modeling – world of tall, well-built, and beautiful people. Each try (“go-see” in modeling jargon) expands my awareness of skillsets and traits to develop to further improve myself just for the sake of being better. The parameter of “being better” is getting projects.  You put on your model gear, face the jury, put away all your issues and just walk. You strut your way on that long ramp,  looking at that spotlight with eyes baring the phrase “I am perfect.” You have to believe it, even just for that time. If you get in, you might be. If not, you’re not yet perfect. “Yet” is the magic word.

I am not perfect today but one day, I might be.

I do not know what urged me to try out this world but what I am certain of is that I have an obsession for self-improvement. Sometimes we are not born with everything, but we are equipped with ways to work our way up one at a time. Slowly, surely, savoring each little progress. Hence, until I adapt to the standards or the standards adjust to me, I will keep trying to better myself. We all have chances and time is on our side. As Einstein puts it, “The essence of time is so that everything doesn’t happen at once.” So until my time comes, I shall improve and further improve. What happens next is beyond what I can control.

The Importance of Proper Posture

Posture is defined as the arrangement of body parts with respect to each other.  Our muscles and bones are designed to be symmetrical; otherwise, muscles can weaken or tighten stressing our joints giving rise to different pain syndromes. Proper posture is our key to avoid body pains and prevent existing ones from worsening.

How can you tell if you have a good posture?

Here is a quick 5-step quick self-assessment you can do:

If you answered NO to any of these questions, one of the major muscles in one side of your body is tight; correspondingly its antagonist muscle (opposite action muscle) is weak.  However, since our muscles are highly trainable, tight muscles can be stretched and weak muscles can be strengthened.

Posture and your Confidence

Looks matter. This is an inevitable fact. Good posture exudes confidence when interacting with different people. Poor posture and poor confidence is correlated and is one vicious cycle. Therefore we must take steps to correct posture slowly so that our confidence will consequentially get a boost.

How to Get the Perfect Power Posture

Let’s simplify the solution into three steps.

Step 1: Workout right. Your F45 trainers are very keen in correcting your posture. Listen to them. Start the unlearning process and re-educate your body to remind it of the ideal posture.

Step 2: Develop awareness. We cannot be with you all the time to remind you of your slouched back or slumped shoulders. Try to take charge of your posture and start the awareness process. Remind yourself, “Bogart, your shoulders.” That is always the start.

Step 3: Modify your environment. This is the hardest part. Heavy workload is not an excuse to damage your body in the long run, but still our environment plays a great role. If everybody in your office is slouched and you have to be the odd one out, initiate and set the standard. Start something. While you are passing that paper to your boss, stand tall. While you are walking to get your caffeine boost, walk smart. On the way to lunch, remind yourself “Bogart, back straight!” That is the beginning.

“Babuuuuu!” exclaims all the kids who grew up in her household. Babu is a Samal-Bangingi word for “mother”. She has nurtured many children – hers and other people’s. I’ve been living under her love and care for two years now and despite our differences in culture and beliefs, I have felt her genuine care that never faltered. We could all attest to her generosity. Behold wisdom from my grandmother, Margarita “Itang” Juan – Cacanando. “Margarita” is Greek for “Pearl”. Indeed, pearl has to be discovered inside a shell that conceals its precious beauty. My lola, our Nanay is hilariously sarcastic and very subtle in her “paglalambing”. Indeed, those slips of thoughtfulness that do not happen regularly will all be very precious to me.

Taft Avenue? Oo! Pumara ka na, baka lumampas ka.

Paano pa malilinis ang Taft Avenue? Nag-aral ako sa Ermita ng limang taon at tinanggap ko na lang ang baho ng hangin doon. Wasak ang napakabangong imahe ko ng kapitolyo ng bansa bilang isang promdi kid na nangarap makapag-aral sa siyudad. Habang sinasaulo ang mga buto at laman ng mga hayop at tao, nilanghap ko ang realidad at tinanggap iyon ng unti-unti hanggang nakasanayan ko na. Wala nang pag-asa, ika ng lahat. Majority wins eh, edi sunod ako sa moda.

Imposible nga kaya? Wala na bang pag-asa? Akala mo lang wala, pero meron! Meron! Meron! Andito na ang Papa, papa cologne! Kaya naman palang bumango ng Maynila para sa isang dakilang bisita. Kaya naman palang pagmukhaing paraiso ang bansa. Dito tayo magaling – magpasikat. Magaling tayong magpasikat at magpa-impress. Sa pagkakataong ito, may mabuti itong naidulot. Walang makakaangal doon. This is something to be proud of. Ituring nating tagumpay ang kakayahan nating sumalubong sa isang mabuting tao, isang bisitang nakakasabik patuluyin sa ating tahanan upang ating makilala at makaumpugang-tuhod. Kaya natin kung gugustuhin. Kaya, diba? Kayang-kaya!

“Malapit na ang bisita, magwalis-walis na!” bulyaw ng ina. Sunod naman ang mga bata, kani-kaniyang hagod at pagpag ng walis tambo. “Andyan na si Ma’am! Andyan na!” sigaw ng class president. Pipirmi naman sa mga kahoy na desk ang mga batang kanina lang ay nagkakantsawan at naghihingian ng baon. Row 1, Row 2, Row 3, lahat “behave”. “Very good, children!” ika naman ng guro bago buklatin ang klas rekord.

Tayo ba ay sadyang mga pa-cute na nilalang? Tayo ba’y ipinanganak na plastik at mapagpanggap? Hindi ko alam. Hindi ko masasagot dahil katulad mo, ganiyan din ang kinalakhan ko. Pantay-pantay tayo, siguro mas matangkad lang ako sayo. Iisang kanin ang kinain natin, pero may extra rice ako. Tulad mo wala akong angal sa adobo. Pareho lang ikaw at ako. Kaya kung susumahin, pantay lang tayo.

Bakit kailangan natin ng bisita para mag-imis ng bahay? Bakit pag nandiyan lang si Ma’am tayo mabait? Bakit pagtalikod ng bisita, magtitinginan tayo at saka siya pagkakatuwaan? Bakit pag-alis ni Ma’am, pagtatawanan at mumurahin natin siya? Ito ba ang ipinagmamalaki nating kultura? Hospitable when you see me, but when you don’t see me now we don’t (#BestInGrammar). Ano kaya talaga tayo? Hindi ko rin alam. Ganiyan din ako eh, pareho lang tayo. Pero ako ay nagtataka; bakit nga kaya? Kung hindi mo ito ginagawa, Mabuhay ka! Kabawasan ka sa problema ng lipunan.

Ngayon, paalis na ang ating Lolo. Maitutuloy kaya ang inudyukan niyang pagbabago? Ewan ko, ewan mo. Basta ako, itutuloy ko ang blog na ito.
